Disclosure of Personal Information
Privacy of your personal information is an essential part
of Tania’s Place providing you with quality services. We understand the importance of protecting your personal
information. We are committed to collecting, using and disclosing your personal
information responsibly. We also
try to be as open as possible about the way we handle your personal information.
It is important to us to provide this service to our Clients and their
Families.

staff members who come in contact with your personal information are aware of
the sensitive nature of the information that you have disclosed to us.
They will all be trained in the appropriate uses and protection of your
information.
In
this consent form, we have outlined what we are doing to ensure that:
- Only
necessary information is collected about your son/daughter and yourself as a
parent and or legal guardian
- We
only share your information with your consent
- Storage,
retention and destruction of your personal information complies with
existing legislation and privacy protocols
- Our
privacy protocols comply with privacy legislation, standards and the law

How Tania’s
Place collects, Uses and Discloses Clients’ and their Families personal
information
Tania’s Place will
collect, use and disclose information about your son/daughter and your family
for the following purposes:

To deliver and to ensure
continuous high quality service

To identify safe and efficient
services and program planning

To implement safe and efficient
services and programs

To assess your son/daughters
health needs for the purpose of appropriate service delivery

To enable us to contact you
(parents or legal guardian)

To establish and maintain
communication with you (parents or legal guardian)

To offer and provide appropriate
programs and services

To allow us to maintain
communication and contact with you (parents or legal guardian) to distribute
program information, updates and confirm registration

To deliver your son/daughters
records to Tania’s Place’s insurance carrier to enable the insurance company
to assess liability and quantify damages, if any

To invoice for services and
programs used
·
To collect unpaid accounts
·
To assist Tania’s Place to
comply with all regulatory requirements

To comply generally with the law
Upon
registration for any programs at Tania's Place we ask that a Client Privacy
Consent Form be signed.
Registration
Refunds
and Cancellation of Services
Our refund policies will vary according to the type of
service you choose. Our commitment is to provide quality programs and
services, however, we recognize that we may not be able to please
everyone. We also ask that families try to be understanding that if
there are Administrative Fess charged they are to cover the expense of having to
continue to pay the staff that were already committed and time to restructure programming. Staff are hired based on the amount of participants and if we
are not notified in a reasonable time period to re adjust staffing, it is
money not well spent and may reflect on the overall rates in the future.
Fees are structured to keep with in family budgets. With any type of
business, if costs are higher than what is being brought in then the consumer
will ultimately be the one who will have to pay. That is a situation WE want
to avoid at all costs.
Day Program
Full and Daily program fees: A request to
Cancel services must be given in writing 2 weeks prior to the cancellation
date. All post dated cheques after the cancellation date
will be returned. The last two weeks would have already been paid therefore no
further fees will be required.
Summer Camp, March Break, P. D. Days, Exam Week,
Christmas Break and Evening Programs: If the cancellation of
services is requested with in the week prior to the requested week/days
registered, there is a full refund. If cancellation is requested during
the week your son/daughter is already participating in the program, the refund
will be pro rated and a 25% Administrative fee will be charged. The
administrative fee will be waived if the cancellation is because of
medical nature, where a Doctors letter is required.
